Inside the Practice: How All for One Physios Use Real-Time Ultrasound in Pelvic Floor Treatment

Pelvic floor issues can feel confusing, frustrating, and—too often—like a guessing game. Are you activating the right muscles? Is what you’re feeling normal? Are those exercises even working? At All for One, we use real-time ultrasound to take the uncertainty out of the equation. It gives both you and your physiotherapist a clear view of what’s happening beneath the surface—so we can treat what’s really going on, not just what it feels like.

For many clients, it’s the first time they’ve truly seen how their pelvic floor is functioning. And that visibility changes everything.

What Is Real-Time Ultrasound?

Real-time ultrasound is a non-invasive imaging tool that allows us to observe soft tissue and muscle function as it happens. Using a handheld device (we use the Butterfly ultrasound), our physiotherapists can assess the pelvic floor muscles externally and watch how they respond to your breath, movement, and conscious engagement.

It’s not diagnostic in the traditional sense—this isn’t about finding pathology. It’s about understanding how your body is currently working and giving you the tools to improve it.

What It Tells Us

Pelvic floor dysfunction can present in many ways: leakage, urgency, prolapse symptoms, pain, heaviness, or difficulty returning to exercise. But those symptoms don’t tell the whole story.

With real-time ultrasound, we can:

  • Assess if you’re actually activating your pelvic floor (and not accidentally bracing your abs, glutes or inner thighs)

  • See the quality of your contraction—how strong it is, how coordinated, how long it can be sustained

  • Identify overactivity—when the muscles can’t fully relax, which is just as common as weakness

  • Evaluate how your breathing, posture, and abdominal pressure influence pelvic floor control

  • Compare left and right sides for symmetry or imbalances

  • Monitor your progress objectively over time

How It’s Used at All for One

Our approach is sensitive, evidence-based, and tailored to your comfort. A real-time ultrasound assessment might look like this:

  1. Consultation & Consent
    Your physiotherapist begins with a full discussion of your symptoms, history, goals, and preferences. You’re given clear information about what ultrasound involves, and it’s only used with your full consent.

  2. Probe Placement
    The ultrasound probe is gently placed on your lower abdomen or, if clinically appropriate, externally over the perineum.

  3. Active Assessment
    You’ll be guided to take deep breaths, engage your pelvic floor, hold a contraction, cough, or perform small movements—all while your physio watches the real-time response on screen. You’ll often be invited to view it too.

  4. Client Education
    Seeing your own muscles at work is a powerful moment. Your physio will explain what they’re seeing and how it relates to your symptoms or concerns. You’ll be guided to practise activating (or relaxing) your pelvic floor in ways that make sense for your body.

  5. Program Planning
    With this insight, your physio will design a treatment plan grounded in what your body truly needs—whether it’s coordination training, tension release, strength building, or return-to-function rehab.

Why It Matters

Research shows that up to 70% of women don’t perform pelvic floor exercises correctly on their own, even with verbal instruction, a significant number still struggle to engage the right muscles. The pelvic floor is hidden, internal, and complex. Without feedback, it’s easy to compensate or brace without realising it.

Real-time ultrasound provides that missing feedback. It closes the loop between intention and action.
