## Creatine, Honestly

**It’s suddenly everywhere, and it’s being recommended for your muscles, your bones and your brain. Most of the enthusiasm is earned. Some of it has run ahead of the evidence. Here’s the honest version.**

If you’ve spent years assuming creatine wasn’t for you, you’re in good company. Most of us did.

For a long time it sat in the supplement aisle looking like it belonged to somebody else, and the things women heard about it were mostly discouraging. It’ll make you bulky. It’ll make you bloated. It’s hard on your kidneys. None of those turned out to be true, but they were repeated for long enough that a whole generation of women simply left it on the shelf.

Now it’s back, and it’s everywhere. Your podcast is talking about it. So is your GP, possibly. It’s being recommended for muscle, for bones, for your brain, for perimenopause, for energy.

So here’s the honest version. What creatine genuinely does, where the excitement has got ahead of the research, and whether it’s worth your money.

## What it actually is

Creatine isn’t a hormone and it isn’t a stimulant. It’s a compound your body already makes and already stores, mostly in your muscles, where it helps supply quick energy for short, hard efforts.

You get some of it from food, mainly meat and fish, and your body produces the rest. Taking it as a supplement simply tops up the tank so there’s more of that fast energy available when you ask for it.

That’s the entire mechanism. Nothing exotic is happening. You’re giving a system you already run a bit more of its own fuel, which is a large part of why it’s so well tolerated and why the research on it is so unusually solid.

## What the evidence genuinely supports

**Strength and muscle, when you’re training.** This is the oldest and strongest finding, and it isn’t close. Alongside resistance training, creatine reliably helps you build strength and lean muscle.

That matters more than it might sound, because building and holding muscle is one of the best things any of us can do for our long-term health, and it gets harder from our thirties onwards. It matters even more through perimenopause and beyond, when muscle and strength decline more steeply. A 2026 analysis pooling results from 608 postmenopausal women found that creatine alongside training added meaningful lean mass and leg press strength compared with a placebo.

The important words there are “alongside training.” Creatine rewards the work. It doesn’t replace it.

**Brain and thinking, promising but early.** This is where a lot of the current excitement sits, and it’s worth being straight with you. Early research suggests creatine may support memory and mental sharpness, particularly in older adults and when you’re short on sleep. A small trial in perimenopausal women showed improvements in reaction time. That’s a genuinely interesting signal, and it’s the reason so many people are paying attention.

It’s also still early. Small studies, not yet repeated at scale. Interesting, not settled.

**Sleep and mood, one to watch.** There are early hints here too, including some evidence that creatine may help alongside treatment for depression, and a study showing longer sleep in menstruating women. Worth following. Not worth planning around yet.

## Where the enthusiasm has got ahead of the evidence

The honesty in the title has to cut both ways, so here are the two claims we’d be careful with.

**Bone.** You’ll see this one a lot, particularly aimed at women over forty, and the evidence is genuinely mixed. The largest and longest trial so far followed postmenopausal women for two years and found no improvement in bone density at the hip or femoral neck. Some finer measurements looked hopeful, but they haven’t been reliably confirmed. Creatine may well help indirectly, by helping you build the muscle and strength that protect bone, but “take creatine for your bones” is ahead of what’s actually been shown.

**Menopause symptoms.** Research in perimenopause and menopause specifically is very new, and it’s encouraging on muscle and possibly on thinking. The leap from there to “creatine helps with menopause symptoms” isn’t supported yet. Promising foundation. Premature promise.

Neither of these is a reason to write creatine off. They’re just where the line sits between what’s proven and what’s hoped for, and that line is exactly where supplement marketing likes to get blurry.

## The myths that kept women away

These deserve to be said plainly, because they cost a lot of women a lot of years.

**It won’t make you bulky.** Creatine helps you build strength and lean muscle in proportion to the training you do. It doesn’t create size on its own, and that kind of bulk is genuinely difficult to build regardless. What you’re building is strength you can use: getting up off the floor, carrying everything in from the car in one trip, keeping up on a hike.

**It probably won’t leave you bloated.** That reputation came from the old loading protocols, where people took large doses for a week. The modern approach skips that, and any extra water is held inside the muscle rather than under the skin.

**It isn’t hard on your kidneys.** In healthy people, long-term studies haven’t found kidney harm. If you have existing kidney disease, that’s a real conversation to have with your doctor first, exactly as it would be with any supplement. For everyone else, this one is a hand-me-down that’s outlived its evidence.

There’s an irony in all of this. The group who spent the longest avoiding creatine may well be the group with the most to gain from it.

## If you decide to take it

The practical part is refreshingly boring, which is usually a good sign.

Look for creatine monohydrate. It’s the original form, it’s by far the most studied, and it’s the cheapest. The pricier “advanced” versions don’t outperform it.

The usual dose is around three to five grams a day, taken consistently. Timing doesn’t matter much. The loading phase you might read about is optional; you’ll arrive at the same place without it, just a little more slowly.

And the part that matters most: creatine works with training, not instead of it. It’s a modest, reliable amplifier of strength work you’re already doing. Sitting in the cupboard, it does very little. Paired with lifting something heavy a couple of times a week, it earns its reputation.

## So is it worth it

For most people who are strength training, yes. It’s inexpensive, it’s safe, it’s better researched than almost anything else on that shelf, and its core benefit, more strength and more muscle from the work you’re already putting in, is the kind of thing that pays off over decades rather than weeks.

Just hold the honest edges in view. The brain research is exciting and early. The bone claims are ahead of the evidence. And none of it does the work for you.

If you’re already training, creatine is one of the few supplements genuinely worth considering. If you’re not, that’s the thing to start with. The supplement can wait.
